Request Access to Your Overlake Medical Records

Overlake Medical Center & Clinics is committed to protecting your medical information by keeping it secure and confidential. Our Health Information Management team can provide you with copies of your medical records. For x-ray or cardiology images, please contact imaging department at 425.688.5564. 

There are several ways to obtain access and get copies of your medical records from Overlake Medical Center & Clinics:

1. Electronic Request

Submit an electronic request for records via our secure online portal.

2. MyChart Account

Request your medical records through your active MyChart account.

3. Forms 

Complete and submit the appropriate forms:

1. Patient Access Request Form: If you are requesting your own medical records, please complete this form, specifying what will be released and to whom.
2. Authorization to Disclose Protected Health Information Form: If a third party is requesting your medical records, please complete this form, specifying what will be released and to whom.

Special Note: Fred Hutchinson Cancer Center Records

Requests for access to Fred Hutchinson Cancer Center records are handled separately. For visits occurring from July 1, 2020, and forward, contact Fred Hutchinson Cancer Center at 425.635.6935 for assistance.

Amendment Requests

If you see documentation in your record that you believe needs correction, please complete this health record amendment form and return it to us at the address listed on the form.

Fees Are Reasonable & Cost-Based

• Records currently stored in electronic record system (MyChart):
  • Delivered to the patient's MyChart account: No charge.
  • Delivered electronically: $6.50.
  • Delivered by paper (printed): $0.90 download + $0.05 per page printing cost.
• Records stored in a paper chart*:
  • Delivered Electronically: $0.07 per page copying cost
  • Printed: $0.07 per page copying cost + $0.05 per page printing cost
• Records stored both electronically and in a paper chart*:
  • Delivered Electronically: $6.50 + $0.07 per page copying cost.
  • Printed: $0.90 download (electronic record) + $0.07 per page copying cost (paper record) + $0.05 per page printing cost.

* Plus sales tax and postage, if applicable.

Note: Records delivered directly to your MyChart account and/or a healthcare provider for continuing care are delivered at no charge.
