How to Notify Overlake of Your Loved One’s Death

We are deeply sorry for your loss. We understand that informing others about the death of your loved one is difficult. Notifying Overlake of your loved one’s passing when it has taken place outside of Overlake helps us close a patient’s electronic medical record and will alert various departments that your loved one has passed. This notification will also help prevent unnecessary communication that may inadvertently cause distress.

Death Certificate as Notification

Ideally, a copy of the death certificate is most helpful to expediting closing the electronic chart and in some cases, it may be a requirement to close the chart.

A death certificate can be hand-delivered, mailed, or faxed to the below address/fax line:

You may also hand-deliver, email, or fax a death certificate to any of our clinics in the area and they are happy to forward it to the Health Information Management department on your behalf.
