Verifying your insurance benefits
Having health insurance doesn’t mean that your plan covers bariatric surgery. Often, coverage is determined by who you work for. Many large local companies provide this benefit to their employees. It is important to confirm with your insurance provider that you have coverage for bariatric surgery. You will need the following CPT codes when contacting your insurance.
- Roux-en-Y Gastric Bypass - CPT Code 43644
- Sleeve Gastrectomy - CPT Code 43775
It is important to note that while the BMI criteria is used by all insurance companies, they may have specific additional requirements for approval of weight loss surgery.
Self-pay options
If your policy excludes bariatric surgery, you may consider reaching out to your Human Resources department to learn more about the benefit options available. Overlake offers a comprehensive self-pay package. This ranges from $19,000-$26,000 depending on the procedure selected. This includes :
- Surgeon and surgical assistant fee.
- Post-op appointments for one year.
- BLISCare Insurance.

BLISCare is an independent insurance that provides a safety net to cover costs of any possible complications associated with bariatric surgery. BLISCare is available exclusively to BLISCare-approved surgeons who meet strict quality standards.
