Monitoring blood sugar levels and sharing the data with your provider is essential to diabetes management.
The following is a list of device resources to help you upload your blood sugars and share other diabetes-related data with our clinic.

Blood Glucose Meters
Blood glucose meters assess blood glucose “sugar” throughout the day as recommended by your provider. The data collected can determine medication-dosing, effects of food, exercise on glucose values, and identify behaviors, which may be contributing to low/high blood glucose.
Instructions for Recommended Devices
Diabetes devices are a great tool towards better diabetes self-management. Many devices can communicate with your cell phone. The instructions/links below can help to determine if your current phone/phone operating system can support the devices listed below and allow you to share data with your care team.
Glooko
Glooko allows you to connect and comprehensively understand your diabetes data from all of your devices: blood glucose meter, CGM, pump and/or smart pen.

Existing Glooko Users
Glooko App Registration
- Go to Glooko website.
- Enter all required fields.
- In the Code field (labeled optional), enter the proconnect code: overlakemc > Click register. This allows for sharing with Overlake Diabetes Education.
Glooko Mobile App Instructions
Mobile App Video Instructions or follow these instructions:
- Download the app > Open the app > Log in with your username and password (used when creating your account) > Sync your devices: blood glucose meter, CGM, pump and/or smart pen.
- Start tracking your diabetes data including: blood glucose, CGM, food, fitness and medications.
Website
- Go to Glooko Uploader.
- Select Mac or PC depending on your computer.
- After the installation file is downloaded, you will need to locate and run that file.
- If you are using a PC: Look in your Recent Downloads folder.
- If you are using a Mac: Look in your Downloads folder in the Finder app.4
- Once you have located the file, double-click on it and follow the on-screen instructions to install the application.
- You will be required to restart your computer at the end of the installation process.
Connecting your devices with a USB Cable
If your devices do not connect via Bluetooth, you can upload from a compatible blood glucose (BG) meter, insulin pump or continuous glucose monitor (CGM) using one of Glooko's upload tools, you will need the appropriate device manufacturer cable.
You may contact the meter manufacturer (phone number will be on the back of your meter) to request a cable to transfer data.

MyChart/Handwritten Logbook
Send a MyChart message to your provider with blood glucose readings added as an attachment by either:
- Take photo(s) of your logbook and attach the images to your message.
or
- Type your blood glucose numbers directly into the body of your message.
Continuous Glucose Monitors (CGM)
Continuous glucose monitoring (CGM) provides real-time glucose values that are checked every five minutes. In addition to glucose value, you may see rate and direction of change as well program alerts for low/high and rise rate/fall rate. These details can help you make decisions related to food, activity and medicine. The data provided by CGM allows you to proactively manage glucose values rather than needing to treat low or high glucose vales when they occur.
Instructions for Recommended Devices
Listed below are continuous glucose monitor devices (CGM). CGMs collect data continuously via a sensor that is placed on either the abdomen or back of the arm depending on approved site location. This data is transmitted to a phone app or separate reader/receiver.
Dexcom
Our clinic will email you a unique share code to receive your data so we have continuous access while also making your visits more efficient. If we have not provided you an invitation to share your data, you can ask your diabetes educator or endocrinologist to send a unique share code to you by sending them a MyChart message. You can stop sharing with our clinic at any time.
To upload and view glucose data and save, print or email reports from home to our clinic:

Log into CLARITY phone app.
- Tap “share” and follow the instructions.

- Log in to the CLARITY website.
- Follow the instructions.
Dexcom receiver
- First-time CLARITY users must create an account.
- Follow instructions to install the Uploader software.
- Connect your receiver to your computer to upload automatically.

Freestyle Libre
Entering our practice ID (see below) into the Libre software system allows us to have continuous access to your diabetes data while also making your visits more efficient. You can stop sharing with our clinic at any time.

Go to Menu > Share > LibreView Connect > Connect to a practice > Enter practice ID 01205713 > Next > Connect.

Insulin Pumps/Smart Pen
Traditional pumps deliver insulin as programmed preset rates determined by the prescribing provider. The user announces pre-meal glucose as well as carbohydrates to be consumed, and the pump will then direct the user how much insulin to deliver or “bolus.” The user then selects deliver bolus at which time they will receive an insulin dose to provide coverage for the meal as well as any correction necessary for pre-meal glucose level.
Insulin pumps consist of a reservoir filled with rapid-acting insulin connected to an infusion set that connects to the body and an internal catheter in which insulin is infused into the body. The most recent advancements in insulin pump therapy are the Artificial Pancreas, Hybrid Closed Loop Models. These pumps have an algorithm within the pump which automate basal rate delivery every five minutes relative to sensor glucose and active insulin in the form of bolus insulin previously delivered.
Tubeless Insulin Pumps
There is a tubeless insulin pump on the market that connects to the body with an internal reservoir filled with insulin. It’s waterproof and can be worn up to three days, providing 72 hours of continuous insulin delivery. It connects with a personal diabetes manager that looks like a smartphone, which is used to program the pump.

Medtronic CareLink
- Set up and upload data from your pump to your computer.
- Send a MyChart message to your provider requesting a Medtronic Carelink invite, if you are not already sharing data with your provider.

Tandem t:connect
Mobile App
- Download the free mobile app by visiting Google Play or the App Store > select t:connect mobile.
- Turn on > mobile connection, unlock the pump > select Options > Device Settings > then select Bluetooth Settings.
- Tap the toggle to turn on the Mobile Connection and enable Bluetooth. *If you don't see Bluetooth Settings, you can visit the Customer Portals to update your pump to the latest software (you may need a prescription and training).
- Log in to mobile app > Use your t:connect web application username and password (or create a login and password in the t:connect mobile app). For best results, upload your pump data to the t:connect web application via a USB cable before starting the next pairing step (USB directions below).
- Pair Insulin pump to mobile app, follow the prompts to pair your t:slim X2 insulin pump with your phone. Only one device can be paired with the t:connect mobile app at a time.

USB Connection
- Set up and upload data from your Tandem pump using the provided USB connection.
- Send a MyChart message to your provider requesting a Tandem t:connect invite, if you are not already sharing data with your provider.

Omnipod/Glooko
Set up and upload data from your Omnipod pump to Glooko software using the provided USB connection to your computer.
- Turn on the Glooko ProConnect Portal on your mobile phone.
- Open the Glooko mobile app. Go to Settings > Account > Add ProConnect code: overlakemc. Then, let you provider or diabetes educator know you have uploaded to Glooko.
- If you are using the original Omnipod (versus the Omnipod Dash), please contact the technical support number below and select option 2 to request a cable free of charge to upload your data.

InPen
Download the free InPen smartphone app:
- Download app > Open the app > Sign up or log in > Click on reports > Generate and download your data.
- Send a MyChart message to your provider or diabetes educator and attach your downloaded InPen report.
