Forms for Patients 

Want to save time at your appointment? You have the option to print patient registration forms and fill them out at home. Just bring the completed forms to your appointment. Many of these are also available on our patient portal, MyChart.

These forms are also available at all Overlake Clinics. If you are a new patient, plan to check in 15 minutes (10 minutes for returning patients) before your scheduled appointment time to complete any paperwork needed. You can also check in and submit your co-pay using MyChart before your visit.

Note: Not every patient needs to fill out each of these forms. When you make your appointment, feel free to ask which forms are needed.
