Because the medical home model is patient-centered, you play a central and significant role. You and your care team are partners, and that means we need your help to give you the best care possible.

Your role includes:

Medical records: Keeping medical information up to date is critical. This includes keeping us informed about your current medications as well as your medical, family and social history. MyChart, our online patient portal, can help with this.

Scheduling: It’s usually easy to know you need a same-day appointment for an immediate need. But it’s also important to schedule regular appointments for preventive care and management of chronic health conditions.

Communication: We can do our best when you keep us informed. Let us know about any medical visits outside of primary care, including visits to urgent care, the emergency room or specialists. That way, we can help you with follow-up care.

We believe you get the best care when it is coordinated across your entire care team. When you partner with us, we can ask the right questions to direct you to the right care.
